What is tirzepatide, and how is it different from semaglutide?

Tirzepatide is the active ingredient in two FDA approved medications. Mounjaro is approved for type 2 diabetes. Zepbound is approved for weight management, and also for moderate to severe obstructive sleep apnea in adults with obesity.

Mostly no, and the rules here have changed a lot. During the shortage that ran from 2022 into 2024, compounding pharmacies were permitted to make copies of tirzepatide. The FDA declared that shortage resolved in late 2024. The grace period ended for 503A pharmacies in February 2025 and for 503B outsourcing facilities in March 2025.

Since then, routine mass produced compounded tirzepatide is no longer permitted. A 503A compounding pharmacy can still prepare it for one specific patient when a prescriber documents a clinical need the FDA approved product cannot meet, such as an allergy to an inactive ingredient. That is a narrow lane, not a loophole, and the FDA has continued to tighten it through 2026.
